    Instructions for using the fillable PDF form below:

    • You must use Adobe Reader to fill out the form. It is free to download and pre-installed on all SLC computers.
    • You cannot use Apple Preview, your web browser's built in PDF reader, or any other PDF viewer.
    • You might have to right-click (Mac users: ctrl-click) and choose "Save as..." to prevent your web browser from automatically opening the form in another viewer.

    Print and sign the form, then return by fax (914-395-2640), mail or hand delivery:
