    If you would like to make an appointment with our on-campus psychiatrist and you have a current mental health provider, please have your provider complete the Mental Health Treatment History Form and return it to the Health & Wellness Center.  If you would like an appointment with our on-campus psychiatrist and do not have a current mental health provider, please call the Health & Wellness Center at 914-395-2350 or go online to schedule a consultation appointment with one of our therapists.
