Guide to SLC Student Mental Health Plan Coverage
Sarah Lawrence College offers a Student Health Plan to all registered students.

The SLC Student Health Plan provides reimbursement at 80% of ‘reasonable and customary rates’ for appointments with any licensed mental health clinician after a $150 deductible per academic year .
When you call for an appointment, ask the clinician if they will bill the insurance company directly.
- If yes, you will be billed the remaining 20% by the clinician, plus any charged amount that exceeds ‘reasonable and customary’ rates.
- If no, you will pay the clinician up front and receive a receipt which you will then submit along with a claim form for reimbursement (see below).

*Itemized receipts should include:
- Student’s name, and date of birth
- Dates of service, all relevant CPT codes, and Diagnostic codes
- Provider license number, address and phone number
