  • Specialized Housing Forms

    All students requesting specialized housing must complete a request form for specialized housing.  Students requesting specialized housing for the first time must also submit a Documentation for Specialized Housing Form which is to be completed by a healthcare professional/provider.

    Students: Please log in to complete the online request form above. Please submit the forms below to your clinician for completion. Previously approved requests do not normally require a new documentation form.

    Specialized Housing is a finite resource and once all Specialized Housing units are allocated, students will be placed on a waitlist.

  • Important Deadline Information

    To ensure your Specialized Housing application is completed in a timely fashion, please submit your Housing Application, Specialized Housing Application, and all appropriate and applicable documentation no later than June 28, 2024.

     

    Please note: late applications will be accepted, but accommodations cannot be guaranteed and you may be placed on a waitlist.
