    The Post-completion OPT guide will provide important information pertaining to what is OPT, Applying for OPT and what happens after you are approved on OPT.

    Request for Post-completion OPT I-20 Form

    Use this form to apply for an OPT I-20 Recommendation for employment authorization after you complete your program. The OPT I-20 allows you to request Post-completion OPT (Optional Practical Training) with the U.S. government (USCIS).

    • Send an email to ISS with all required documents.

    To use the OPT Request Form:

    • You must Download the form and use Adobe Reader (free) to fill out the form.
    • You cannot use Apple Preview, your web browser's built in PDF reader, or any other PDF viewer.
    • You might have to right-click (Mac users: ctrl-click) and choose "Save as..." to prevent your web browser from automatically opening the form in another viewer.
