Commonly Asked Questions and Answers for Incoming Students

  1. Can I get an extension for the deadline on submitting my  forms?

The Health & Wellness Center cannot officially offer extensions to the Colleges published due date for the required health forms. If you do not meet the deadline  to submit ALL forms, your record will be designated as ‘non‐compliant’ and you will receive email notifications indicating this. These email notifications are automated. Your checklist will also indicate that you are ‘non‐compliant’ and you will remain on 'registration hold' (blocking you from registering for classes) until you are compliant with ALL required health forms. Once you have submitted everything, and your forms have been reviewed and approved by the Health & Wellness Center, you will be ‘compliant’ in the College’s computer system and your ‘registration hold’, as described above, will be removed. Please allow time for staff to review forms.

 

  1. Why are forms that I uploaded showing up as non-compliant?

Once all required forms are submitted, you will remain non-compliant until all forms are verified by our staff. To ensure timely verification, please review your forms to ensure you are submitting all that is required. Once everything is verified, you will show as being in compliance. If there are any concerns regarding submitted documents, you will be contacted. Please be sure to check your SLC email regularly as this is how we will contact you.

 

  1. Can I submit records in another language?

Please have all records translated into English prior to submission. This helps to ensure accurate and timely verification of your records. 

 

  1. Can I use my own provider’s immunization and health forms?

We kindly ask that you use our student health forms as it encompasses all of our requirements for incoming students. This also helps to ensure timely verification of your submitted forms. 

 

  1. I can’t find the health forms that I need to submit. Where do I go to find them?

All necessary forms are on the MySLC checklist for incoming students. For newly admitted Undergraduate, Graduate, and Human Genetics students please go to the following site to access forms:

  1. What if I only have 1 (one) MMR vaccination?

Please see our Immunization Fact Sheet included in your required health forms for further details on the Measles, Mumps, and Rubella required vaccinations.  Immunization Fact Sheet

Students born on or after January 1, 1957 are required to have two doses of live vaccine for Measles, one dose of live vaccine  for Mumps, and one dose of live vaccine  for Rubella.   To satisfy this requirement if you only have 1 MMR vaccination, you can do the following:

  1. Obtain a second MMR vaccination at least 28 days apart from the first MMR vaccination. 

  2. Obtain serological proof of immunity through blood antibody titers  to Measles, Mumps, and Rubella showing POSITIVE immunity. 

 

  1. I completed the  Tuberculosis (TB) risk factor screening and as per the screening need to  have a PPD skin test or IGRA test. What forms of IGRA testing are acceptable?

Two acceptable forms of IGRA testing are Quanteferon-TB Gold or T-spot blood test.

 

  1. I’m a Human Genetics Graduate Student-do I need titers for everything?

If you have provided the required immunization information, you will be compliant with the College. However, titers are required for clinical rotations. 

 

  1.  How do I seek a medical or religious exemption for the required immunizations?

  2. Any student seeking a medical or religious exemption, must read and sign page 1 the RELIGIOUS OR MEDICAL EXEMPTION FROM IMMUNIZATION REQUIREMENTS FORM. 

  3. Any student seeking a medical exemption, must also have a licensed healthcare provider (non-family member) complete  page 2 the Request for Medical Exemption From Immunization Form and upload both page 1 and 2 to your portal for review. 

  4. Any student seeking a religious exemption, must complete the Request for Religious Exemption From Immunization Form on page 3 and upload both page 1 and 3 to your portal for review. 

 

  1.  I have questions about my health insurance and coverage at SLC?

  1. How do I cancel  the student health insurance plan?

All students enrolled in Sarah Lawrence are automatically enrolled in the student health plan. This request is processed with  the student accounts office at Sarah Lawrence College. All students get billed for the Student Health Insurance Plan (SHIP).  If you have comparable coverage that covers you in New York you can waive the SHIP by going to www.universityhealthplans.com to complete an insurance waiver.  Once the waiver is processed, the charge will be removed from your bill. You have the option to opt out of the insurance and use your existing insurance by filling out a waiver form. In order to waive the insurance, they have to have a current insurance plan that is comparable to the student health insurance. Kaiser Permanente insurance and Blue Cross Blue Shield’s New England is not considered comparable in order for the waiver to be completed.

       12.  I have questions about the wellness fee and my tuition bill?

            If you choose the waive the student health insurance the health insurance fee will be removed from your bill. The wellness fee is separate and covers a range of wellness related services at Sarah Lawrence College including the free visits within the centers scope of practice at the center for both medical and counseling services, the crisis hotline, and wellness programming. Please note there are no-show fees and other small fees for certain items at Health & Wellness.

      13. How do I establish care locally to SLC for a medical or a psychological condition?

 If you are looking to establish care with a local provider for either medical and/or psychological conditions, we recommend first checking to see which providers are “in network” with your insurance plan. We have a list of local medical and psychological groups that we frequently refer our students to of  which can be found on the following page:Local Resources
