*Please Note: 

  • First Year students who are non-compliant with the required health forms, along with all students (First and Second Year) who have not provided a COVID vaccine record must meet with the Office of Health & Wellness before they can get their Sarah Lawrence ID card.

  • All students who have a financial hold on their account must meet with and be cleared by Student Accounts prior to getting their Sarah Lawrence ID card.

Complete the online/hybrid/in person confirmation form to let us know whether you plan to engage in your program online, in a hybrid format, or in person in fall 2021 (if you are not studying fully in person, be sure to discuss and confirm your plans with your program director):

Medical Forms

Complete all required medical forms, including your COVID-19 vaccinations, and submit those forms to Health and Wellness.

If vaccines are not available in your country of residence, you are not yet eligible to be vaccinated, you received a non-approved vaccine, and/or you will not be fully vaccinated by July 8, please reach out to the Health & Wellness Center (healthservices@sarahlawrence.edu) to discuss your situation to ensure you can receive an extension
