Below you can find links to the forms used most by graduate students.  If you cannot find what you need or have any questions, please contact graduateadministration@sarahlawrence.edu.

Master's Degree Application Form (Online form)

This form is to be filled out by students who intend to graduate December 2022, May 2023, or August 2023.

Independent Study Proposal 

This form should be completed if you wish to do an independent study.

Fieldwork Proposal

Use this form to propose your fieldwork.

Deadline:  End of the first week of classes.

Add/drop Form

Use this form to add/drop a class during the add/drop period. Note you should have discussed with your faculty and your advisor. 

Audit Request

The audit request form is only available during the audit request period, after add/drop. You can find the form on the registration page, under "forms", at this time. Note you should have discussed with your faculty and your advisor. 

Request for an Incomplete

Use this form to request an incomplete.

Request to Withhold Directory Information

Use this form.

CHANGE OF STATUS FORMS*

Leave of Absence

  • If the student and/or the program director needs to discuss a potential leave of absence with the Dean of Graduate & Professional Studies, they should reach out to the Dean and highlight key questions and/or the need to meet to discuss prior to completing an official Leave of Absence Request Form
  • Once it has been decided that a leave of absence is the best course of action for the student and the leave has been approved by the program director, the student completes a Leave of Absence Request Form.  This form must be signed by both the student and the program director.  
  • International students must also meet with the international graduate student advisor,  Shirley Be (sbe@sarahlawrence.edu), to go over their F1 status regulations and have her sign the Leave of Absence Request Form before it is submitted to the Office of Graduate Studies.
  • Once the form is completed, the student then sends the form to the Office of Graduate Studies by email at gradadministration@sarahlawrence.edu.

Return from Leave of Absence

  • Prior to registration, a student should inform the Office of Graduate Studies if they are intending to return from a leave of absence by completing a Return from Leave of Absence Form.  This form should be signed by the program director. 
  • International students must also meet with the international graduate student advisor,  Shirley Be (sbe@sarahlawrence.edu), to go over their F1 status regulations and have her sign the Return from Leave of Absence Request Form before it is submitted to the Office of Graduate Studies.
  •  The student then sends the completed, signed form to the Office of Graduate Studies by email at gradadministration@sarahlawrence.edu .

Withdrawal

  • If a student wishes to withdraw from the college, they must notify the program director stating their intention.  

We highly recommend that all students wishing to withdraw from the college meet with their program director to discuss their desire to withdraw.

  • After notifying and meeting with their program director, students who wish to withdraw should complete a Withdrawal Form. This form must be signed by the student and the program director.
  • International students must also meet with the international graduate student advisor,  Shirley Be (sbe@sarahlawrence.edu), to go over their F1 status regulations and have her sign the Withdrawal Form before it is submitted to the Office of Graduate Studies.
  •  The student then sends the completed, signed form to the Office of Graduate Studies by email at gradadministration@sarahlawrence.edu .
  • Students with a federal loan must also complete an exit interview with Beverly Roach-Esprit  in the Financial Aid Office

Full Time to Part Time

  • If a student wishes to go from full time to part time, they must complete a Full Time to Part Time Form that they and the program director must sign. 
  • The student should meet with the financial aid office (Beverly Roach-Esprit) to discuss how their part time status will affect their financial aid awards and have her sign the form as well. 
  • International students must also meet with the international graduate student advisor,  Shirley Be (sbe@sarahlawrence.edu), to go over their F1 status regulations and have her sign the Full Time to Part Time Form before it is submitted to the Office of Graduate Studies.
  • The student then sends the completed, signed form to the Office of Graduate Studies by email at gradadministration@sarahlawrence.edu 

Part Time to Full Time

  •  If a student wishes to go from part time to full time, they must complete a Part Time to Full Time Form that they and the program director must sign. 
  • The program director should refer the student to the financial aid office (Beverly Roach-Esprit) to discuss how their new full time status will affect their financial aid awards and have her sign the form as well.
  • International students must also meet with the international graduate student advisor,  Shirley Be (sbe@sarahlawrence.edu), to go over their F1 status regulations and have her sign the Part Time to Full Time Form before it is submitted to the Office of Graduate Studies.
  • The student then sends the completed, signed form to the Office of Graduate Studies by email at gradadministration@sarahlawrence.edu .
